Lung changes during hexamethonium therapy for hypertension.

چکیده
During the course of treating 54 severely hypertensive patients with parenteral hexamethonium bromide, wehave observed the development of unexpected pulmonary dyspnea and associated radiographic changes in the lungs in three cases after 7, 9, and 12 months' therapy respectively. Two of these came to autopsy; the third has recovered. The object of this paper is to record the clinical, radiological, and pathological aspects of these pulmonary changes. Complications such as disabling headaches, cardiac enlargement, or renal damage were present in all cases in this series, and in seventeen the disease was in an acute malignant or pre-malignant phase. The dose of hexamethonium required varied from 50 mg. to over 600 mg. three times daily. The blood pressure was controlled at levels 30-90 mm. Hg systolic and 10-40 mm. Hg diastolic below the pre-treatment averages. The three patients here recorded all required large doses of hexamethonium, but others who received as much or more of the drug for periods up to 27 months have shown no lung changes.

The treatment of hypertension with hexamethonium.
Hexamethonium by subcutaneous injection in doses of 10 to 75 mg. of the ion every 8 or 12 hours produced significant reductions of arterial pressure and symptomatic improvement in a high percentage of patients with benign or malignant hypertension. Methods for preventing tolerance and undesirable side effects are presented, and the advantages as well as the limitations of hexamethonium therapy ...
متن کاملNeural contribution to renal hypertension following acute renal artery stenosis in conscious rats.
To assess the hemodynamic changes during acute renal artery stenosis (RSt) and their dependence on alterations in the renin-angiotensin and sympathetic nervous systems, we studied conscious rats chronically instrumented with miniaturized pulsed-Doppler flow probes. Probes were implanted on the superior mesenteric and both renal arteries, and on the lower abdominal aorta for measurement of mesen...
متن کاملOcular effects of methonium compounds.
SINCE the demonstration by Paton and Zaimis (1948) of the ganglionic blocking activity of the pentamethonium and hexamethonium compounds, the pharmacological action of these drugs in man has been fairly extensively studied, and trials have been made of their clinical use in arterial hypertension and duodenal ulcer. However, only brief and scattered mention has been made of the ocular changes re...

Neurogenic (autonomic) factors in hypertensive vascular disease (1, 2) may be studied with hexamethonium 2 which reduces arterial blood pressure by blockade of the sympathetic ganglia. When the blood pressure is reduced acutely in this manner, alterations in renal hemodynamics have been observed. Following a single dose of hexamethonium administered parenterally to patients with hypertension, t...
متن کاملBlood pressure during exercise and the effect of hexamethonium.
This paper is concerned with the effect of exercise on the blood pressure and the modification of this effect by hexamethonium bromide. It is well known that the methonium compounds can produce a fall in the blood pressure ofmany hypertensive subjects when lying down. This fall, however, is variable and inconstant. In normal persons, the methonium compounds do not produce a fall in the basal bl...
